What you need before you start

Before you open the form, have these four things ready. It makes the difference between finishing in one sitting and getting stuck halfway through:

• Passport details — the exact name and passport number as printed. Do not abbreviate middle names; the form expects them exactly as they appear.
• Arrival flight number and airline — check your booking confirmation, not your memory. A wrong flight number is the most common cause of a rejected declaration.
• Address where you will stay in Jamaica — the full address of your hotel, resort or host for the first night. A resort name alone is not enough; the form asks for the physical address and parish.
• Email address and phone number — use one you check while travelling. Your confirmation and any updates go there.

Common mistakes that delay approval

These are the issues that come up most often. Avoiding them saves a round trip:

• Name order: entering given name and surname in the wrong fields. The form mirrors your passport exactly.
• Flight date vs arrival date: selecting the departure date instead of the arrival date in Jamaica.
• Group bookings: completing one form for a whole family. Each passenger needs their own C5 declaration, including children and minors.
• Accommodation address: typing the booking reference instead of the physical address and parish. The confirmation number is not a location.

Yes. Every air passenger arriving in Jamaica must submit the C5 Immigration and Customs declaration before going through immigration, including children and minors.

How long does the review take?
